Four Community organizations are working together and are improving the lives of 15 patients.

So says The LHIN’s Director of Community Engagement.

Cynthia Stables says the local LHIN, North Bay Regional Health Centre, People For Equal Partnership in Mental Health and Nipissing Mental Health Housing and Support Services have combined their services to help transition patients from hospital to the community.

She says these patients are now living in permanent housing or at a transitional residence where they have access to services.

Stables says there are many benefits for patients if they live in a community setting.

Stables says there’s a cost benefit too with a $3-million annual savings to the system when people are living in the community as opposed to staying at the hospital.

The CEO of the North Bay Regional Health Centre says the LHIN has made investments that is creating a win-win: people are healthier and happier because they are living independantly in the community and the hospital is focusing on patients with high needs
